Remux "M2V + AC3 + subtitles" => VOB

When I have demuxed the VOB with Smartripper to:

- M2V video
- AC3 audio
- RAWSUB subtitles

I would like to remux those files back to the VOB file to burn them to a DVD-R.

Which tools do you use to:

- Demux the subtitles from VOB files
- Remux the M2V, AC3, subtitles
