Also, after having made an operation in PgcEdit that modifies the VOB files (such as blanking a domain, a PGC or a cell, or adding a cell), don't forget to save the DVD. Otherwise, the VOBs and the IFOs will be desynchronized. The error you got might be caused by this kind of misuse.

Saving after after each modification (kill)? Thats something I've never done before. I usually save at the end before I close the app.

No, you don't need to save each time, but you must save your session. The problem is that the VOB file is modified immediately on HDD, but the IFOs are modified only in memory. To synchronize them, they must be saved.
Take care at the power failures, too!
Whenever a VOB has been modified but the IFOs are not saved, use FixVTS or VobBlanker to remove the new, unreferenced VOB cells.
