I recently downloaded R4me with the installer and got Menushrink in the package. How does it work within R4me, does it automatically respond if the size is too big?

No. You will have to set it up as a tool. The guide shows you how to set up tools.
Or you can use it separately. There is a help file with MenuShrink.
