adding your own subtitles......?????

Hi I just want to know how i should do to add my owmn subtitles with dvdshrink.
For exampel: I have The Patriot as an region 1 (usa)movie and i live in sweden and i want to add swedish subtitles so i can see the movie with swedish subtitles.
i know how to ripp and compress and burn but i dont know how to add my own subtitles.....help me pleas anyone........
