Write Error... what causes the Gap in the burned dvd?

Write Error... what causes the Gap in the burned dvd? I recently tried to make a copy of Something New, and I got a write error, and then i looked at the bottom of the disc, and there was a big gap between a part that was burnt and the continuation of that burn... basically it looked like 3 ring bullseye... what causes this error.. Thanks for ur help in advance

crappy media, PIO mode, burning too fast with a combination of these etc. what software was used to burn with?

I must've had it too fast, i was burning at 12x.... I use Verbatim dvd+r...and I was using Nero Express 6.6, thank u for the reply... I was thinking maybe my burner was dying... that would suck...
