subtitle creator/inserter?

Does anyone know of a tool to create/insert subtitles or closed captioning into and MPEG2 for DVD? All I've seen here rae tools to rip subtitle info.

You can use Sub-Station Alpha to create a subtitle script, though I have no idea how you can input it into MPEG-2 files.
