what to put in DVD credits

Not sure if this is an appropriate forum to seek answers to my questions, but....
I have finished a promotional DVD for a friends furniture design business, I have used all original imagery and footage that we shot. However i'm not sure what to put in the end credits as far as produced by... and edited by... etc, but mainly copyright protection.
Is there any specific info to put there or some kind of standard template to follow.
Any help would be greatly appreciated.

The short answer is that it is your DVD production - put in whatever you like!

Why would you want to copyright what is essentially an ad and which your friend would presumably want circulated far and wide?

I don't know if your work is actually considered copyrighted if you simply place a notice in your credits. If you want real copyright protection for your work in the US you need to register with the US copyright office Don't know of any premade templates, but you can find alot of info here.
