TSCV and chapters

I sometimes have problems creating chapters in TSCV. I get a message that's something like this "chapter in same sector as previous one..." (something like that) and that the chapter will be placed the same place as the last one. I recently made a vcd where none of the chapters worked. Why does this happen? Is there a way to "fix" this?
